This service is available through both the NDIS (except NSW) and Aged Care
Having good social connections is essential for all aspects of your health, especially as you aged. People First Healthcare can support you to improve your social connections and connect with your community for your wellbeing. An outing or a chat over a cup of coffee with a caring and friendly volunteer could become a highlight of your week, and give you the confidence to meet new people.
These services are for people who are not in regular social contact with other people.
People First Healthcare social connection programs can support you and link you to other people and your community. Programs include home visits, social outings, group activities, peer support and social phone calls.
Many programs will match you with a volunteer based on your interests and values, to be your companion so you can build up a friendship over time.
Activities might include:
"Thank you People First Healthcare for providing professional services for those who need that extra support to live life to its fullest. It’s nice to find a team that really cares for its clients and their well-being, while providing the best care possible. Thank you PFH team!"
"Each and every carer was kind, gentle and cared for me in the very best of ways. My case manager excelled in her duties. People First Healthcare have enabled me to maintain my independence in my own home these past years although I was completely wheelchair bound. I am ever so grateful."
