Most of our Liverpool work starts at the hospital
Liverpool Hospital is the major referral and trauma centre for south-western Sydney, and that shapes the enquiries we get from 2170. People here don't usually end up there for something small. It's a fractured hip, a stroke, a cardiac event or a chest infection that turned into three weeks on a ward.
Then the phone call: she's medically ready to go home, what have you got in place? Families are often blindsided by it, because a month earlier nothing was needed and nobody had heard of an assessment. Ring us the day discharge is first mentioned rather than the day before it happens.
What the first fortnight home actually needs
Usually three things rather than a lot of hours. In-home nursing covers the clinical tail of the admission: dressings, changed medications, catheter or diabetes management. Skipped or misread medication is one of the most common reasons someone is back on the ward inside a month.
An occupational therapist through allied health should see the house in the first week, and they're looking at unglamorous things: the step at the back door, the height of the bed, whether the shower has anything to hold on to. Whatever that turns up feeds into home modifications. Then personal care, because the first shower at home after a hip replacement is where a person either gets their confidence back or decides they can't do it anymore.
Around those, domestic assistance and meals and nutrition keep the house running while someone is still weak, and care management handles the coordination if the family is stretched.
The 2170 catchment
2170 is a large postcode and we work across it: Liverpool, Warwick Farm, Casula, Moorebank, Chipping Norton, Lurnea, Ashcroft, Miller, Prestons and Green Valley. Liverpool is four stops from Fairfield on the Cumberland line, through Canley Vale, Cabramatta and Warwick Farm, so it's a straightforward run from our office at 60 Ware Street.
When driving stops
Central Liverpool is compact if you're well. Macquarie Mall, Westfield, the station and the hospital sit within a short walk of each other. None of that holds with a walking frame, a recent stroke, or a specialist appointment at nine in the morning in mid-July. Transport is one of the most requested services we run here.
Languages
Liverpool's older residents include large Arabic-speaking and Assyrian communities, Serbian and Croatian speakers who came out decades ago, growing Hindi and Punjabi-speaking families, and Vietnamese and Spanish speakers. The place this bites hardest is the bedside handover. Discharge instructions get delivered quickly, in English, to a tired family who nod because that's what you do. A week later nobody is certain which of the four boxes replaced the old blood pressure tablet. A carer or nurse who can go through it again in the right language solves a problem that looks administrative and isn't.

Call us the day discharge is first raised rather than the day before it happens. Privately funded care can usually be arranged fastest. If she has an approved Support at Home package, care generally starts within a week of the agreement being signed. Either way, tell us on the phone what the ward has said about wounds, medication and mobility, because that decides whether the first visit is a nurse or a carer.

Can a carer take Dad to his outpatient appointments at Liverpool Hospital?
Yes, that is what our transport service is for. A carer collects him from home, gets him into the building, waits and brings him back. For most people the hard part is not the distance, it is the walk from the car park and the hour in the waiting room.
Do you have Arabic or Assyrian speaking carers in Liverpool?
Both are among the languages we are asked for most in 2170, along with Hindi, Punjabi, Serbian, Vietnamese and Spanish. Ask when you call. It matters most straight after a hospital stay, when medication instructions given quickly in English at the bedside are the thing most likely to be misunderstood.
What if we do not have a package yet?
You can start privately and switch over later. Support at Home replaced Home Care Packages on 1 November 2025 and an assessment takes time, which is a problem when someone is coming home next week. Nothing stops you beginning with privately funded care and moving onto the package when it is approved.
