Home from Sunshine Hospital

Sunshine Hospital is on Furlong Road and, despite the name, it is in St Albans. Locals know that; people arranging care from interstate usually do not. It is Western Health's main campus for this side of Melbourne, so if someone in 3020 has had a fall, a stroke or a hip, that is the campus they will have come out of.

The two weeks after a discharge are where most home care arrangements are won or lost. The medication list has changed. There is a wound. The physio in the ward said to keep walking and nobody has walked since. In-home nursing takes the dressings and the medication management, allied health continues the physiotherapy so the ward gains do not drain away, and an occupational therapist looks at the house before the first fall rather than after the second.

Houses built for the Harvester years

Sunshine grew around industry, and the housing reflects it: brick and weatherboard on solid blocks, put up between the fifties and the seventies for families working locally. Many of those houses have never changed hands. The people who bought them are now in their eighties, and the block that made the place worth buying has become the reason the family is worried.

General home maintenance is unfashionable and it does more good here than most things: the mowing, the edges, the gutters, the light bulb that requires a ladder nobody should be on. Domestic assistance handles the inside, which in a large old house takes longer than a scheduling system thinks it should.

Vietnamese, Maltese, Italian

Sunshine and St Albans have one of the larger Vietnamese communities in Melbourne, layered over the Maltese and Italian families who arrived a generation earlier and stayed. Walk Hampshire Road and you can hear it.

What that means for care is simple. An eighty-eight-year-old who came here in 1962 and worked in English for forty years may not be working in English any more, particularly if there is dementia in the picture. A carer who can talk to her is doing a fundamentally different job to one who is talking at her. Say which language you want when you ring. We match where we can, and where we can't we say so rather than send someone and hope.

The spouse who has become a carer

In a suburb with this little turnover, a lot of the people we visit are couples who have been in the same house for half a century. When one of them declines, the other takes over: the showering, the medications, the getting up in the night. They rarely call it caring. It is just what you do, and it goes on until the well spouse is exhausted or unwell themselves.

Respite is the service that keeps those arrangements standing. A carer sits with your husband while you sleep, or go to the doctor yourself, or attend a funeral. Where memory is part of it, dementia care brings routine and a familiar face rather than a rotating cast of strangers.

Getting about without the car

Sunshine station is one of the better-connected places in the west, with the Sunbury line and the V/Line services to Ballarat, Bendigo and Geelong all passing through, and Sunshine Marketplace is close to it. None of that helps an eighty-year-old carry two bags of shopping up a platform ramp. Transport covers the appointments and the shopping runs, door to door, with the bags carried in. Personal care covers the mornings, which is where most families start.
